  • Guidelines for Traditional Chinese Medicine Diagnosis and Treatment of Depressive Disorders

    Depressive disorders are mainly characterized by persistent depressed mood,with a high prevalence,recurrence rate and disability rate. The goals of traditional Chinese medicine(TCM) in treating depressive disorders include improving the cure rate,quality of life and preventing recurrence,emphasizing comprehensive attention to both physical and psychological factors. However,the diagnosis and treatment guidelines for depressive disorders in TCM mostly start from the differentiation of zang-fu organs,while the research on the diagnosis and treatment of psychological disorders is still insufficient. Therefore,our team,based on clinical practice and through discussions with experts,has proposed and optimized measures for the treatment of depressive disorders with TCM and reached an expert consensus,in order to provide a better reference for clinicians to rationalize and standardize TCM treatment,so as to improve the cure rate of patients with depressive disorders and promote the rapid recovery of patients with depressive disorders.

  • Application of Traditional Chinese Medicine System Theory Principles in Treatment of Oculomotor Nerve Palsy of Qi and Blood Deficiency Type

    Traditional Chinese medicine(TCM) system theory is a modern theory based on the crossdisciplinary research of TCM and system science,which is used to solve the increasingly complex problems related to multi-system,multi-factor,and multi-pathogenesis in modern clinical practice. This article systematically expounds the organic and functional principles of TCM system theory,pointing out that the organic principle emphasizes the interaction and overall correlation between various elements inside and outside the human body system,while the functional principle reveals the dynamic process of diseases from functional disorders to structural changes. This article takes oculomotor nerve palsy of “qi and blood deficiency” type as an example,combines the method of systematic sphygmology for pattern differentiation and analyzes the process flow of the patient from qi and blood depletion to loss of meridian nourishment and ultimately leading to abnormal ocular function. It explores the pathogenesis of abnormal function of A-structural abnormality-abnormal function of B,and emphasizes the core of overall tonification of qi and blood and regulation of function A in the treatment in combination with therapeutic methods such as strengthening the spleen and eliminating dampness,warming the meridians and unblocking the collaterals,it achieves functional repair from the overall to the local level. It aims to demonstrate the advantages of TCM systems theory in analyzing the mechanisms of complex diseases and guiding systematic treatment,providing theoretical and clinical references for modern TCM in dealing with complex diseases involving multiple systems and factors.

  • Analysis of Characteristics of Qilu Typhoid Fever School in Differentiating and Treating Difficult and Complicated Diseases

    The academic characteristics and diagnostic and therapeutic thinking of the Qilu Typhoid Fever School have advantages in the diagnosis and treatment of difficult and complicated diseases. This paper takes the academic viewpoints,diagnosis and treatment techniques of advantageous diseases,and medication patterns of Master LI Keshao and his representative inheritors as the research contents. Through literature research,data mining,analysis and comparison,and induction and summary,Qilu Typhoid Fever School's essential,core,and consistent characteristics of diagnosing and treating difficult and complicated diseases are refined as follows:adhering to the law for typhoid fever,integrating learning and application;keeping the mind free of distration and prioritizing syndrome differentiation;combining disease and syndrome,integrating traditional Chinese medicine with Western medicine;regulating the spleen and stomach,differentiating and treating difficult and complicated diseases;differentiating chronic and strange diseases based on phlegm and blood stasis;making good use of classic prescriptions and drafting new prescriptions on one's own;varying dosage forms and mixing and matching for treatment;being logically rigorous and attaching great importance to thinking. Master LI Keshao's distinctive academic ideas established the basic model for Qilu Typhoid Fever School's differentiation and treatment of difficult and complicated diseases. Although JIANG Jianguo,DING Yuanqing,YU Junsheng,and SI Guomin are proficient in treating different diseases as representative inheritors,they all inherit Master LI Keshao's medical conduct and clinical experience in treating various difficult and strange diseases.

  • Exploration on Correlation between “Spleen Acting as the Protector of the Body” and Tumor Immunity from Perspective of Intestinal Mucosal Immune Homeostasis

    The pathogenesis of tumors is very complex. Modern research suggests that their occurrence and development are closely related to the immune function of the body. Regulating the function of the immune system is a key approach of tumor prevention and treatment. Intestinal mucosal immunity is an important component of the immune system,which can affect the prognosis of diseases and intervene in the therapeutic effect. Traditional Chinese medicine(TCM) holds that “ spleen acting as the protector of the body”,which is a highly summarized description of the spleen's defense function through the medium of defensive qi. The immune site of the intestinal mucosa is located in the intestine,and its function belongs to the spleen,which is highly consistent with the theory in TCM that “spleen acting as the protector of the body”. The imbalance of intestinal mucosal immune homeostasis is similar to the pathological transformation process of “spleen acting as the protector of the body-the spleen failing to act as the protector of the body”,and “the spleen failing to act as the protector of the body” may be the mechanism of tumor occurrence in TCM. This article takes “spleen acting as the protector of the body” as the entry point to explore the correlation and clinical application between the imbalance of intestinal mucosal immune homeostasis in tumors and “the spleen failing to act as the protector of the body”,in order to provide a theoretical basis for the prevention and treatment of tumors from the perspective of “treating the spleen and regulating the protector of the body” to restore intestinal mucosal immune homeostasis.

  • Exploration on Application of Lung-spleen “Tongqi Xiangqiu” in Polymyositis/Dermatomyositis Complicated with Interstitial Lung Disease

    The etiology of polymyositis/dermatomyositis combined with interstitial lung disease has not been clarified. Combined with the relevant traditional Chinese medicine(TCM) classics,it is believed that its clinical manifestations can be related to the lung and spleen in the five viscera of TCM. The theory of “origin-analogue” plays an important role in TCM. This paper expounds the “origin-analogue” relationship between lung and spleen from the aspects of zang-fu function,meridians and five elements. Based on the theory of “origin-analogue”,the etiology and pathogenesis of the disease are explored from the two aspects of exogenous pathogen invasion and lung and spleen injury,and corresponding treatment methods are proposed in different stages(acute attack stage and remission stage),providing theoretical support for the TCM treatment of polymyositis/dermatomyositis combined with interstitial lung disease,and providing new ideas for the clinical treatment of the disease.

  • Exploration on Effect of Bile Acid-Intestinal Microbiota Axis on Intrahepatic Cholestasis Based on “Earth Congestion and Wood Stagnation”

    Intrahepatic cholestasis is a common liver system disease. The imbalance of the bile acid-intestinal microbiota axis is closely related to the occurrence and development of intrahepatic cholestasis. “Earth congestion and wood stagnation” is a pathological process in which the spleen earth is obstructed and counterattacks the liver wood,leading to the stagnation of liver wood. It plays a significant role in the pathogenesis of intrahepatic cholestasis. Intestinal microecological disorder and bile acid metabolism disorder are important manifestations of “earth congestion and wood stagnation”. This article explores the relationship among “earth congestion and wood stagnation”,bile acid-intestinal microbiota axis imbalance,and intrahepatic cholestasis,and proposes that the bile acid-intestinal microbiota axis can be intervened based on the “earth congestion and wood stagnation” theory to prevent and treat intrahepatic cholestasis,which provides a new idea for the integrated prevention and treatment of intrahepatic cholestasis with traditional Chinese and Western medicine.

  • Analysis of Medicinal Properties of Shigao in Zhuye Shigao Decoction(竹叶石膏汤)

    The debate over whether the medicinal property of Shigɑo(Gypsum Fibrosum) is slightly cold or extremely cold has persisted for a long time. Impacted by the heat-clearing effect of Baihu Decoction(白虎汤)and the record of Shigɑo being extremely cold in Treatise on the Medicinal Substances by Famous Physicians,later generations generally believe that Shigɑo used in Treatise on Febrile and Miscellaneous Diseases is an extremely cold substance. Nevertheless,Shennong's Herbal Classic of Materia Medica recorded that the medicinal property of Shigɑo was slightly cold. Through the analysis of the 8 prescriptions in Treatise on Febrile and Miscellaneous Diseases for treating fever and containing Shigɑo,it is discovered that Shigɑo is mostly not used for clearing heat;although it is used for clearing heat in the prescriptions of the Baihu Decoction type,it mainly assists Zhimu(Rhizoma Anemarrhenae). Based on the premise that the medicinal property of Shigɑo is slightly cold,the function of Shigɑo in Zhuye Shigao Decoction(竹叶石膏汤) is re-analyzed,it is found that Shigɑo in Zhuye Shigao Decoction mainly serves to suppress adverse rising and stop vomiting. Fever is not an essential syndrome of Zhuye Shigao Decoction,and it can expand the clinical application of Zhuye Shigao Decoction in post-recovery,cough,and vomiting,etc.

  • Clinical Study on Rumo Huangji Formula(乳没黄及方) Chinese Herbal Fomentation Combined with TDP Irradiation in Treating Patients with Postoperative Perianal Edema of Mixed Hemorrhoids with Damp-heat and Blood Stasis

    Objective:To analyze the clinical efficacy of Rumo Huangji Formula(乳没黄及方) Chinese herbal fomentation combined with specific electromagnetic spectrum(TDP) irradiation in the treatment of postoperative perianal edema of mixed hemorrhoids with damp-heat and blood stasis,and its effect on the anal function and pro-angiogenic factors of patients. Methods:A total of 140 patients with mixed hemorrhoids with damp-heat and blood stasis who were treated in our Hospital from June 2023 to June 2024 were prospectively selected and divided into a control group and an observation group by using the random number table method,with 70 cases in each group. The control group was treated with TDP irradiation combined with warm water sitz bath after the operation,while the observation group was treated with Rumo Huangji Formula Chinese herbal fomentation combined with TDP irradiation after the operation. The clinical efficacy,traditional Chinese medicine syndrome score,anal function and pro-angiogenic factors,inflammatory factor levels,and adverse reactions of the two groups were compared. Results:After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05). After treatment,the TCM syndrome scores,the longest contraction time of the anal canal(ALCT),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),and C-reactive protein(CRP) levels of the two groups of patients were all lower than those before treatment. The levels of anal systolic blood pressure(ASP),anal diastolic blood pressure(ADP),anal resting blood pressure(ARP),vascular endothelial growth factor(VEGF),fibroblast growth factor-1(FGF-1),fibronectin(FN),and transforming growth factor-β1(TGF-β1) were all higher than those before treatment(P<0.05). Moreover,the levels of TCM syndrome score,ALCT,TNF-α,IL-1β,IL-6 and CRP in the observation group were lower than those in the control group,while the levels of ASP,ADP,ARP,VEGF,FGF-1,FN and TGF-β1 in the observation group were higher than those in the control group(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusions:Rumo Huangji Formula Chinese herbal fomentation combined with TDP irradiation can improve the levels of angiogenic factors and inflammatory factors,alleviate clinical symptoms such as anal edge edema,enhance the anal function of patients,and promote their recovery in the treatment of patients with perianal edema of mixed hemorrhoids with damp-heat and blood stasis.

  • Clinical Observation on Tongfeng Pill(痛风丸) Combined with Febuxostat in Treating Patients in Remission Period(Phlegm Turbidity and Blood Stasis Syndrome)

    Objective:To observe the clinical efficacy of the Tongfeng Pill(痛风丸) combined with febuxostat in the treatment of gout in the remission period. Methods:A total of 60 patients with gout in the remission stage with phlegm turbidity and blood stasis syndrome in traditional Chinese medicine(TCM) were included and randomly divided into the observation group and the control group by using the random number table method,with 30 cases in each group. The control group was treated with oral febuxostat,while the observation group was treated with Tongfeng Pill combined with febuxostat. The treatment period was 12 weeks. The recurrence rate of gout during the treatment period was observed;Blood uric acid(BUA) levels before and after treatment,inflammatory indicators levels[C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)],Visual Analogue scale(VAS) for pain,and TCM syndrome score were compared;The changes in tophi area,the volume of joint effusion and the infrared thermal aberration value between the affected side and the healthy side of the joint were detected by musculoskeletal ultrasound before and after treatment. Results:After treatment in both groups,the recurrence rate was 53.30% in the control group and 36.70% in the observation group. The Log-rank test showed a statistically significant difference(P<0.05). Compared with that before treatment,the relative baseline changes of BUA,CRP,ESR,VAS and TCM syndrome score in the control group were smaller than those in the observation group,and the differences between the groups were statistically significant(P<0.05). Tophi area size,joint effusion volume and infrared thermal aberration values in both groups improved compared with those before treatment,with greater improvements in the observation group. The effective rate of the control group was 76.70%,and that of the observation group was 86.7%. The efficacy grade of the observation group was higher than the control group(P<0.05). Conclusions:Compared with the treatment with Western medicine alone,Tongfeng Pill combined with febuxostat has a lower risk of recurrence in the treatment of gout in the remission period(phlegm turbidity and blood stasis syndrome). It can effectively reduce blood uric acid levels and relieve clinical symptoms,and has good safety. It is expected to prevent and treat hyperuricemia and prevent gout recurrence,and is worthy of promotion.

  • Clinical Study on Buyi Ganshen Formula(补益肝肾方) Combined with Acupuncture on Elderly Patients with Degenerative Lumbar Spinal Stenosis

    Objective:To observe the clinical efficacy of Buyi Ganshen Formula(补益肝肾方) combined with acupuncture on elderly patients with degenerative lumbar spinal stenosis. Methods:A total of 102 elderly patients with degenerative lumbar spinal stenosis were selected and divided into the acupuncture group and Buyi Ganshen Formula combination group by using random number table method,with 51 cases in each group(1 case dropped out in the acupuncture group,1 case dropped out and 1 case was excluded in the Buyi Ganshen Formula combination group). Both groups were given basic treatment simultaneously. The acupuncture group received acupuncture treatment on this basis,while Buyi Ganshen Formula combination group received administration of Buyi Ganshen Formula on the basis of the treatment of the acupuncture group. Both groups of patients were treated continuously for one month. The levels of C1q/TNF-related protein 3(CTRP3),tumor necrosis factor-α(TNF-α),and monocyte chemoattractant protein 1(MCP-1) in patients before and after treatment were observed. Microcirculation indicators of patients were detected before and after treatment:Hematocrit,plasma viscosity,fibrinogen level,the distance of neurogenic claudication of the patients were recorded. Before and after treatment,the lumbar pain and lumbar function of the patients were evaluated by visual analogue scale(VAS) for pain,roland morris disability questionnaire(RMDQ),and oswestry disability index(ODI). The traditional Chinese medicine syndrome score and the clinical efficacy were compared between the Buyi Ganshen Formula combination group and the acupuncture group. Results:After treatment,the total effective rate of patients in the Buyi Ganshen Formula combination group was 97.96%,and that in the acupuncture group was 84.00%. The efficacy in the Buyi Ganshen Formula combination group was better than that in the acupuncture group(P<0.05). The levels of TNF-α and MCP-1 in both groups were lower than those before treatment(P<0.05),and the levels of TNF-α and MCP-1 in th Buyi Ganshen Formula combination group were lower than those in the acupuncture group(P<0.05). After treatment,the levels of CTRP3 in both groups were higher than those before treatment(P<0.05). The level of CTRP3 in the Buyi Ganshen Formula combination group was higher than that in the acupuncture group(P<0.05). After treatment,the hematocrit,plasma viscosity,and fibrinogen levels in both groups were lower than those before treatment(P<0.05),the hematocrit,plasma viscosity and fibrinogen levels in the Buyi Ganshen Formula combination group were lower than those in the acupuncture group(P<0.05). After treatment,the ODI,RMDQ,and VAS scores of both groups were lower than those before treatment(P<0.05),and the ODI,RMDQ,and VAS scores of the Buyi Ganshen Formula combination group were lower than those of the acupuncture group(P<0.05). After treatment,the distance of neurogenic claudication in both groups increased compared with that before treatment(P<0.05),and the distance of neurogenic claudication in the Buyi Ganshen Formula combination group was longer than that in the acupuncture group(P<0.05). After treatment,the scores of soreness and pain in the waist and legs,emaciation and shortness of breath,weakness in the legs and knees,and total scores in both groups were lower than those before treatment(P<0.05),and those in the Buyi Ganshen Formula combination group were lower than those in the acupuncture group(P<0.05). Conclusions:Buyi Ganshen Formula combined with acupuncture can regulate the levels of serum inflammatory mediators,improve the microcirculation of the spinal canal and neurogenic claudication,relieve lumbar pain,enhance lumbar function and clinical efficacy in the treatment of elderly patients with degenerative lumbar spinal stenosis.

  • Observation on Efficacy of Tongue Acupuncture Combined with Ultrasound Therapy in Patients with Swallowing Disorders after Radiotherapy for Nasopharyngeal Carcinoma

    Objective:To study the effect of tongue acupuncture therapy combined with ultrasound on swallowing function of patients with dysphagia after radiotherapy for nasopharyngeal carcinoma. Methods:Sixty patients with dysphagia after radiotherapy for nasopharyngeal carcinoma who visited Cangzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2022 to May 2023 were selected and randomly divided into the control group(conventional rehabilitation and ultrasound treatment) and the observation group(conventional rehabilitation,ultrasound therapy,and tongue acupuncture therapy) according to the order of treatment,with 30 cases in each group. The duration of the swallowing period before and after treatment in the two groups of patients was determined by television fluorescence swallowing study(VFSS). The water swallowing test(WST) grades of the two groups of patients before and after treatment were evaluated by using the WST test. The clinical efficacy was valuated based on the changes in WST grades. The swallowing function of the two groups of patients before and after treatment was evaluated by using the Chinese version of the Anderson Dysphagia Scale(MDADI). Results:After treatment,the durations of the oral phase,swallowing reflex,pharyngeal phase,laryngeal closure phase in both groups decreased. Moreover,the observation group were greater than those in the control group,and the differences were statistically significant(P<0.05). After treatment,the WST grades of the two groups of patients were better than those before treatment,and the observation group after treatment were better than those of the control group. The clinical efficacy of the observation group after treatment was better than the control group. MDADI score in both groups decreased,the observation group was greater than those in the control group. Conclusions:Tongue acupuncture therapy combined with ultrasound treatment can reduce the time required for swallowing food ball,liquids,and coughing during swallowing in patients with dysphagia after radiotherapy for nasopharyngeal carcinoma,as well as improve the swallowing function and enhance the quality of life.

  • Experience of Traditional Chinese Medicine Master ZHANG Zhiyuan in Treating Arthralgia Syndrome

    This article summarizes Professor ZHANG Zhiyuan's experience in treating arthralgia syndrome. Based on the classics such as The Yellow Emperor's Canon of Internal Medicine and Synopsis of the Golden Chamber,Master ZHANG proposes new pathogenesis theories such as “yin deficiency arthralgia”,“arthralgia caused by disharmony of ying and wei”,and “arthralgia caused by obstruction of the fine collaterals in Xuanfu” on the basis of the traditional theory of arthralgia caused by wind,cold,dampness and heat. He summarizes the characteristic therapeutic methods of nourishing yin to relieve arthralgia,promoting the circulation of qi and unblocking collaterals,harmonizing ying and wei,inducing sweating to relieve arthralgia,clearing heat and promoting diuresis,and warming yang to relieve arthralgia and accumulates distinctive medicinal experience such as “Shengdihuang(Rehmannia Raw) nourishing yin and relieving arthralgia”,“Huangqi(Astragalus Membranaceus) inducing sweating and relieving arthralgia”,“Shigao(Gypsum Fibrosum) clearing heat and relieving arthralgia”,and “Duhuo(Angelicae Pubescentis Radix),Fengfang(Vespae Nidus) and Dahuang(Rhei Radix ET Rhizoma) unblocking collaterals and promoting the circulation of qi to treat arthralgia”.

  • Traditional Chinese Medicine Master WANG Xinlu's Experience in Treating Myasthenia Gravis Based on Theory of Blood Turbidity

    This article systematically reviews the academic thoughts and clinical experience of Professor WANG Xinlu,a master of traditional Chinese medicine(TCM),in diagnosing and treating myasthenia gravis based on the theory of blood turbidity. Myasthenia gravis can be classified under the category of flaccidity disease in TCM,and blood turbidity is an important pathological factor for its occurrence and development. Professor WANG Xinlu believes that the four core pathogenic mechanisms of the disease are spleen turbidity and yang stagnation,turbidity transforming into heat,blood turbidity blocking the collaterals,and turbidity damaging the spleen and kidneys. He proposes four methods:combining turbidity transformation with strengthening the spleen and ascending yang qi,clearing heat and nourishing the extremities,unblocking yang and dredging the collaterals,and warming the kidneys and benefiting the marrow. In clinical practice,the diagnosis and treatment of the underlying disease based on blood turbidity,integrating various methods flexibly often yields remarkable results,which provides a new approach for TCM in treating this disease and bridging the gap between Chinese and Western medicine.     Professor PENG Peichu differentiates and treats mucosal lesions based on the theory of “pathogenic factors accumulating in Sanjiao and dysfunction in acting as a pivot”. He believes that the mucosa is similar to the functions of Sanjiao in terms of distribution,protection of the body,and maintenance of metabolic processes,and is similar to the function of the Shaoyang in protecting the body and communicating between the inside and outside. Once the diet is not balanced,causing pathogenic factors to accumulate in Sanjiao and internal damp-heat obstruction,or emotional distress leading to liver and gallbladder stagnation and dysfunction of the Shaoyang pivot,all of which can lead to the occurrence of mucosal lesions. Based on this,the basic pathogenesis of mucosal lesions is summarized as “pathogenic factors accumulating in Sanjiao and dysfunction in acting as a pivot”. In treatment,the principle of “clearing heat,promoting diuresis,unblocking Sanjiao,soothing the liver and gallbladder,and harmonizing the pivot” is followed,and a Nianmo Formula(黏膜方) is prescribed to achieve the effects of harmonizing the Shaoyang,clearing heat and promoting diuresis,and unblocking the bowels and eliminating turbidity. It has achieved remarkable results in the treatment of mucosal lesions such as chronic atrophic gastritis,oral ulcers,and bladder cancer,with a prospect of providing inspiration for the diagnosis and treatment of mucosal lesions.

    Professor GU Enpeng believes that gouty arthritis is rooted in the deficiency of the spleen and stomach,resulting in phlegm,dampness and blood stasis,which blocks the circulation of qi and blood in the meridians,leading to stagnation and internal heat. It is often triggered by alcohol toxicity,fish and crab toxicity. During an attack,redness,swelling,heat and pain are present simultaneously. The treatment should start from the spleen and stomach in the middle Jiao. When the middle Jiao is active,phlegm,dampness and blood stasis will be eliminated,and it is also necessary to clear heat and remove blood stasis. When the spleen is healthy,the acquired qi will be sufficient. It is also necessary to nourish the liver and kidneys. When the spleen qi is abundant,the distribution of body fluids is in a proper way. Coupled with the tonification of qi and nourishment of blood,the body's healthy qi is restored,pathogenic factors are eliminated,and diseases tend to heal. Acupuncture is used to regulate meridians,promote the transformation and transportation of the spleen and stomach in the middle Jiao,and resolve phlegm,dampness and blood stasis. External application is used to clear heat,reduce swelling and relieve pain. Diet is used to regulate the imbalance of the body and prevent recurrence.     Gastroesophageal reflux disease(GERD) is one of the common digestive system diseases characterized by typical symptoms such as heartburn and reflux. Professor LV Wenliang believes that the pathogenesis of this disease is primarily characterized by damp-heat obstructing the middle Jiao,with spleen and stomach deficiency as the root cause,and liver stagnation and earth dampness as the secondary factors. This disease is caused by damp-heat,food accumulation,qi stagnation,phlegm-dampness obstruction in the middle Jiao,loss of the ascending and descending functions,imbalance in the middle Jiao,and the upward movement of turbid qi rebelling upwards into the esophagus. In terms of treatment,based on the principle of “treating the middle Jiao as a balance”,the physiological characteristics of the mutual dependence and complementarity of the spleen and stomach are always taken into account. The priority is to clear heat and eliminate dampness,with the modified formula of Lianpu Decoction(连朴饮). At the same time,it is combined with methods such as tonifying qi and strengthening the spleen,aiding digestion and eliminating food stagnation,soothing the liver and harmonizing the stomach,applying medicinals with both pungent and bitter flavors,cold and warm properties,attacking and tonifying simultaneously,and regulating the liver and stomach in harmony.   • A Case Report on Treatment of Pemphigoid Nodularis from Perspective of “Qi,Blood,and Body Fluids” in Dermatology

    This article reports a case of treatment of nodular pemphigoid,a rare skin disease,based on the theory of qi,blood,body fluids differentiation in dermatology and the theory of earth dampness and wood stagnation. The main cause of this case is congenital intolerance,excessive five emotions,and earth dampness and wood stagnation. The core pathogenesis is the combination of wind and dampness with qi and blood,leading to the transformation of heat into toxins that are hidden in the skin and muscles. Symptomatic treatment in acute condition,treatment focuses on relieving symptoms using formulas such as Chushi Zhiyang Decoction(除湿止痒汤),Qingre Chushi Decoction(清热除湿汤) and Quanchong Formula(全虫方) to clear and detoxify dampness. Radical treatment in chronic cases,treatment focuses on addressing the root cause using formulas such as Chaihu Guizhi Ganjiang Decoction(柴胡桂枝干姜汤) and Chaihu Shugan Powder(柴胡疏肝散),combined with nourishing medicinals like Shengdihuang(Rehmanniae Radix) and Duzhong(Eucommiae Cortex) to warm the spleen earth,ascend the liver qi,and harmonize the yin and yang of the kidneys.

    Depression,as a multi-dimensional mental disorder,is characterized by its core clinical manifestations of persistent emotional regulation disorder,lack of motivation and cognitive function impairment. It has become one of the mental health problems with the heaviest disease burden worldwide. Traditional Chinese medicine(TCM) preparation,with their unique advantages of multiple ingredients and multiple targets,have demonstrated significant potential in antidepressant. However,the complexity of the ingredients of TCM and the highly selective permeability of the bloodbrain barrier limit its targeted delivery to the brain and clinical transformation. The novel brain-targeted delivery system significantly enhances the bioavailability and brain-specific characteristics of active ingredients in TCM. This article reviews the role of TCM preparation in the treatment of depression and the influence of the blood-brain barrier in the treatment of depression.   • Research Progress of Traditional Chinese Medicine in Protecting Schwann Cells from Diabetic Peripheral Neuropathy by Inhibiting Inflammatory Responses

    Diabetic peripheral neuropathy(DPN) is the main cause of lower extremity amputation and disabling neuropathic pain. Schwann cells(SCs) are the main glial cells in the peripheral nervous system and play a crucial role in the repair after nerve injury. Inflammatory response damage to SCs is an important factor in the occurrence and development of DPN. At present,the treatment of DPN has not yet achieved the desired effect. Traditional Chinese medicine(TCM) has achieved better clinical results in the prevention and treatment of DPN with its unique advantages of multi-targets and multi-pathways. TCM effectively inhibits the apoptosis of SCs and improves neurological function by regulating the nuclear factor kappa-B(NF-κB) signaling pathway and the Nod-like receptor protein 3(NLRP3) inflammasome. This paper summarizes the intervention of TCM in inhibiting inflammatory response and intervening in the apoptosis of SCs from the aspects of single traditional Chinese medicinal and compound active ingredients,with a prospect of providing a reference for the integrated traditional Chinese and Western medicine prevention and treatment strategies of DPN.
