  • Technical Specifications for Pulse Diagnosis of Systematic Sphygmology for Pattern Differentiation

    Systematic sphygmology for pattern differentiation is a brand-new pulse science system that follows the fundamental principles and laws of systems theory,integrates clinical experience,and has unique insights. It encompasses multiple disciplines and covers multiple levels. It focuses on the development and differentiation of finger sensory functions,the establishment of a “scenario memory system” for various pulse element sensations,and the diagnosis and examination of objective “evidence chains” during the disease process,demonstrating characteristics such as systematicness,scientificity,standardization,practicality and ease of operation. However,the absence of operational specifications has hindered the clinical promotion and application as well as further innovative development of the pulse diagnosis technique of systematic sphygmology for pattern differentiation. This article,based on existing research findings,related literature on systematic sphygmology for pattern differentiation,and expert consensus,standardizes the techniques of systematic syndrome differentiation and pulse diagnosis from aspects such as pulse palpation,search and identification pulse information,analysis and generalization pulse information,aiming to promote the popularization and innovative development of the pulse diagnosis technique of systematic sphygmology for pattern differentiation.

  • Exploration and Analysis of Development and Application of Theory “The Diseases That This Meridian Is in Charge of Treating” of the Six Yin Meridians

    Miraculous Pivot·The Channels puts forward the theory that “the diseases that this meridian is in charge of treating” in describing the twelve meridians,which holds that diseases of the six yin meridians are closely related to the dysfunction of the internal organs. The article analyzes the characteristic of “the diseases that this meridian is in charge of treating” of the six yin meridians,pointing out that the “the diseases” mainly manifest as functional disorders of the corresponding internal organs. It further elaborates on the principle of acupuncture treatment that “treating the internal organs by selecting their corresponding shu acupoints”,emphasizing the core position of the five-shu acupoints in treating internal organ disorders. At the same time,by integrating the medical records of past dynasties with modern clinical research,the specific plans and therapeutic effects of using the five-shu acupoints of the six yin meridians to treat diseases of the respiratory,digestive,cardiovascular,urogenital and other systems are systematically summarized,and the clinical value of this theory in guiding the diagnosis and treatment of meridian-zang-fu organ related diseases is verified.

  • Clinical Study on Premenstrual Acupuncture and Moxibustion Stimulation of Ren Meridian in Treatment of Primary Dysmenorrhea

    Objective:To evaluate the efficacy and safety of premenstrual acupuncture and moxibustion stimulation of the Ren meridian in the treatment of primary dysmenorrhea(PD). Methods:This study adopted a prospective cohort design,with the exposure factor being whether the Ren meridian was stimulated. Eventually,108 patients with primary dysmenorrhea who received acupuncture and moxibustion treatment were included. According to the actual situation of the subjects receiving acupuncture and moxibustion treatment,they were divided into the Ren meridian group(73 cases) and the non-Ren meridian group(35 cases). The acupuncture method and acupoints are determined based on the patient's condition. The treatment was continued for three menstrual cycles,followed up for one menstrual cycle thereafter. The COX Menstrual Symptom Scale(CMSS) scores(including severity and duration) during the treatment period and the follow-up period were compared between the two groups. The scores of various traditional Chinese medicine(TCM) syndromes and the total scores of the two groups before and after treatment were compared. The short-term clinical efficacy and the clinical efficacy during the follow-up period of the two groups were compared. Results:During the first,second and third menstrual cycles of treatment and the follow-up period,the CMSS scores(severity and duration) of both groups decreased compared with those before treatment(P<0.05). With the advancement of the treatment course,the CMSS scores(severity and duration) of both groups showed a decreasing trend(P<0.01). During the follow-up period,the CMSS score(severity and duration) of the Ren meridian group was lower than that of the non-Ren meridian group(P<0.05). Repeated measures analysis of variance was used to test the CMSS score data. The results showed that there was an interaction between exposure factors and time factors(P<0.05),and both were important influencing factors of the CMSS score. However,there was no statistically significant difference in the analysis of the effects between the two subjects(P>0.05). After treatment,each TCM syndrome score and total scores of both groups decreased compared with those before treatment(P<0.01). After treatment,the TCM syndrome scores of whether there were blood clots,breast distension and pain,and scanty menstruation and total scores in the Ren meridian group were all lower than those in the non-Ren meridian group(P<0.05). There was no statistically significant difference in the short-term clinical efficacy between the two groups(P>0.05). The therapeutic effect of the Ren meridian group during the follow-up period was better than that of the non-Ren meridian group(P<0.05). Conclusions:Premenstrual acupuncture and moxibustion stimulation of the Ren meridian has demonstrated excellent efficacy and high safety in the treatment of primary dysmenorrhea,providing high-quality real-world evidence for the clinical treatment of primary dysmenorrhea.

  • Exploration and Analysis of Functional Subdivisions of Sanjiao and Effects of Acupuncture

    This article systematically analyzes the scientific connotations of Sanjiao qi transformation and water channels functions,clarifies their intrinsic connection with the meridian system and the regulation mechanism of acupuncture. It is proposed that Sanjiao is a unique system in the theory of Zangxiang in traditional Chinese medicine,constructs the macroscopic architecture of human life activities through the spatial compartmentalization of “three regions” and the physiological operation of “two functions”. In terms of spatial distribution,Sanjiao are vertically arranged as the upper jiao being like mist(dispersing and diffusing),the middle jiao being like fermentation(digesting and ripening),and the lower jiao being like a ditch(excreting). Functionally,their dual roles of “transporting the primordial qi”(qi transformation) and “circulating body fluids”(water channels) correspond respectively to the physiological mechanisms of energy and material metabolism and body fluid distribution in Western medicine. Sanjiao and the meridian system are interdependent and complementary,jointly facilitating the circulation and distribution of qi,blood and body fluids. From the perspective of modern biomedicine,the macroscopic functions of Sanjiao can be mapped to the neuro-metabolism-immune network. Acupuncture exerts a therapeutic effect of “regulating the pivot to facilitate functional activities” by modulating key nodes within this net-work(such as nerve conduction,mitochondrial energy conversion and immune cell polarization).

  • Exploration on Sanren Decoction(三仁汤) in Treatment of Keratotic Eczema Based on Unblocking and Harmonizing Sanjiao

    Based on the theory of Sanjiao qi transformation,this article expounds the etiology and pathogenesis of keratotic eczema. It is believed that the etiology and pathogenesis of the disease are due to the obstruction of Sanjiao qi transformation :Firstly,the distribution and metabolism of body fluids are blocked,leading to malnutrition and impeded dispersion of the skin,resulting in dry and cracked skin;Secondly,the blood circulation is not smooth,and the blood stasis blocks the meridians,leading to the hyperplasia and thickening of local skin lesions. Thirdly,the skin's smooth flow is abnormal,the pathogen has no way out,and thus the condition recurs. It is proposed to diagnose and treat keratotic eczema from the perspective of unblocking and harmonizing Sanjiao,and the Sanren Decoction(三 仁 汤)is selected,of which the clinical effect is remarkable,providing certain ideas for the prevention and treatment of keratotic eczema. A case of keratotic eczema on the hand is attached for vertification.

  • Warming Kidney and Promoting Qi Transformation Method in Treatment of Overactive Bladder

    Overactive bladder(OAB) is a common functional disease of the urinary system,which has the characteristics of unknown etiology and recurrent attacks,affecting the quality of life and even physical and mental health of patients. The core pathogenesis of OAB is the dysfunction of qi transformation in the kidney,bladder and Sanjiao caused by deficiency of kidney qi. In clinical practice,the method of warming the kidney and transforming qi should be adopted,warming and consolidating the bladder,combining tonification and astringency,and integrating warming and relieving constipation,tonifying the kidney,soothing the liver and strengthening the spleen. The main medication is warming and tonifying the spleen and kidney,solidifying essence and constricting urine. The prescriptions are based on Gongdi pills(巩堤丸),Jinsuo Gujing pills(金锁 固 精 丸) and Suoquan pills(缩 泉 丸),and modifications are made. Acupuncture and medication are combined,and the medicinals of nourishing qi and promoting blood circulation and relieving constipation are combined to make tonifying without stagnation.   • Differentiation and Treatment of Primary Myelofibrosis Based on “Consumptive Disease-Dry Blood-Chronic Infectious Disease”

    The formation of blood stasis begins with the deficiency of qi and blood due to consumptive disease,which leads to the lack of source and poor circulation of qi and blood. Deficiency and weakness,as well as blood stasis and dryness,generate dry blood. Subsequently,due to the steam and dampness of pathogenic heat,it evolves into chronic infectious disease,thus forming a transmission and transformation path of “consumptive disease-dry blood-chronic infectious disease”. Based on the dominant position of the pathological elements of blood stasis in the generation,development and evolution of primary myelofibrosis,this article explores the pathogenesis and syndrome differentiation and treatment of primary myelofibrosis. It holds that the onset of the disease is based on internal deficiency and fatigue,the formation of the disease is often due to the internal formation of dry blood,and the transformation of the disease is due to internal consumption,which leads to chronic infectious disease. The treatment of this disease should take into account both strengthening the body's resistance and eliminating pathogenic factors. In the early stage of fibrosis,the basic treatment lies in strengthening the body and clearing the source. During the obvious fibrosis stage,the treatment should focus on gradual recovery and tonifying deficiency. When the disease progresses and turns into leukemia,the key is to nourish yin and detoxify.   • Exploration and Analysis of Treatment of Prediabetes Mellitus Based on “Dispersion Can Relieve Congestion”

    “Dispersion can relieve congestion” refers to the treatment of congestion by promoting the circulation of qi through dispersing medicinals. This theory emphasizes promoting the circulation of qi,eliminating congestion,and restoring the normal body function. The main cause of prediabetes mellitus(PDM) lies in the obstruction of qi movement in the middle jiao and the accumulation of damp-heat. The treatment requires restoring the qi movement in the middle jiao,and promoting the circulation of qi is a key strategy. Starting from the theory of “dispersion can relieve congestion”,this article classifies PDM into three syndrome types based on the intermingled characteristics of qi congestion,heat congestion and dampness stagnation :qi congestion combined with heat congestion,qi congestion combined with dampness congestion,and damp-heat combined with congestion. It proposes that the pungent and bitter medincals should be combined on the basis of the combination of property and flavor,combined with the therapeutic methods of unblocking the fu organs,promoting diuresis,aromatherapy and guiding stagnation,the proportion of medicinals should be adjusted according to the main contradiction to make congestion follow qi flow and congestion be eliminated to ensure smooth qi flow. In light of the relatively rigid status of PDM treatment,future research should start from the idea of combining pungent and bitter medincals and further explore the formulation techniques and medicinals selection of various schools in the Jin,Yuan,Ming and Qing Dynasties to optimize the treatment plan.

  • Clinical Study on Shihu Xiaowei Decoction(石斛消萎汤) Combined with Standard Quadruple Therapy in Treatment of Chronic Atrophic Gastritis Complicated with Gastric Intestinal Metaplasia

    Objective:To explore the effects of Shihu Xiaowei Decoction(石 斛 消 萎 汤) combined with standard quadruple therapy on intestinal flora,oxidative stress and inflammatory factors in patients with chronic atrophic gastritis(CAG) complicated with gastric intestinal metaplasia(GIM). Methods:A total of 110 CAG patients complicated with GIM were selected and randomly divided into the western medicine group(treated with standard quadruple therapy) and the integrated traditional Chinese and western medicine group(treated with Shihu Xiaowei Decoction combined with standard quadruple therapy) according to the random number table method,with 55 cases in each group. The intestinal flora indexes,oxidative stress indexes,inflammatory factors,gastric function indexes,and gastric mucosal pathological scores before and after treatment were compared between the two groups. In addition,the clinical efficacy and incidence of adverse reactions were compared between the two groups. Results:After treatment,the levels of Lactobacillus,Bifidobacterium and superoxide dismutase(SOD) in both groups increased significantly compared with those before treatment,and the levels in the integrated traditional Chinese and western medicine group were higher than those in the western medicine group(P<0.05). The levels of Enterococcus,Enterobacter,nitric oxide(NO),and malondialdehyde(MDA) were all lower than those before treatment,and the levels in the integrated traditional Chinese and western medicine group were lower than those in the western medicine group(P<0.05). After treatment,the levels of interleukin-6(IL-6),Creactive protein(CRP),and tumor necrosis factor-α(TNF-α) in both groups were lower than those before treatment,and the levels in the integrated traditional Chinese and western medicine group were lower than those in the western medicine group(P<0.05). After treatment,the levels of pepsinogen I(PG I) and pepsinogen ratio(PGR) in both groups increased compared with those before treatment,and the levels in the integrated traditional Chinese and western medicine group were higher than those in the western medicine group(P<0.05). The level of pepsinogen II(PG II) was lower than that before treatment,and the level of the integrated traditional Chinese and western medicine group was lower than in the western medicine group(P<0.05). After treatment,the GIM,gastric mucosal atrophy and mucosal inflammation scores of both groups were lower than those before treatment,and the scores of the integrated traditional Chinese and western medicine group were lower than those of the western medicine group(P<0.05). The total effective rate of treatment of the integrated traditional Chinese and western medicine group(94.55%) was higher than that of the western medicine group(80.00%)(P<0.05). The total incidence of adverse reactions in the integrated traditional Chinese and western medicine group was 7.27%,which was lower than 23.64% in the western medicine group(P<0.05). Conclusion:Shihu Xiaowei Decoction combined with standard quadruple therapy has a better therapeutic effect in the treatment of CAG complicated with GIM. It can inhibit oxidative stress response and inflammatory factors,maintain the balance of intestinal flora,protect gastric mucosa,improve gastric function indexes and reduce the incidence rate of adverse reactions.

  • Effects of Sangzhu Yangzheng Herbal Tea(桑术养正饮) on Immune Function and Oropharyngeal Microorganisms of Population after SARS-CoV-2 Infection

    Objective:To observe the effects of Sangzhu Yangzheng Herbal Tea(桑术养正饮) on the immune function and oropharyngeal microorganisms of population after SARS-CoV-2 infection. Methods:Sixty subjects infected with the SARS-CoV-2 from December 7,2022 to December 31,2022 were recruited and divided into a control group and a traditional Chinese medicine group according to the random number table method,with 30 cases in each group. The traditional Chinese medicine group was treated with oral administration of Sangzhu Yangzheng Herbal Tea,while the control group received no intervention. The observation period was 2 weeks. The levels of T lymphocyte subsets(including the counts of CD3~+ T cells,CD4~+ T cells,and CD8~+ T cells) before observation and on the 14 th day after observation were compared between the two groups. 16S rRNA sequencing was performed on throat swab samples of patients in the traditional Chinese medicine group before and after treatment respectively to evaluate the diversity and composition of oropharyngeal microbiota,and to conduct microbiota difference analysis. Results:On the 14 th day after observation,the levels of CD3~+ T cells,CD4~+ T cells and CD8~+ T cells in the traditional Chinese medicine group all increased compared with those before treatment(P<0.05). The levels of CD3~+ T cells,CD4~+ T cells and CD8~+ T cells in the control group decreased compared with those before observation(P<0.05). On the 14 th day after observation,the levels of CD3~+ T cells,CD4~+ T cells and CD8~+ T cells in the traditional Chinese medicine group were higher than those in the control group(P<0.01). After treatment,the Alpha diversity(Ace,Chao and Sobs indices) in the traditional Chinese medicine group was all increased compared with that before treatment(P<0.05). The composition of the microbiota at the phylum level was mainly composed of Firmicutes,Bacteroides,etc. At the genus level,dominant genera such as Streptococcus,Rothia,and Prevotella accounted for a significant proportion. After treatment,the relative abundance of Prevotella,Isprevotella,Cilium,Catococcus and Peptostreptococcus in the traditional Chinese medicine group increased compared with that before treatment(P<0.05). No obvious adverse reactions occurred in either group. Conclusion:Sangzhu Yangzheng Herbal Tea can improve the T-cell immunity of people after SARS-CoV-2 infection,improve the structure of oropharyngeal microorganisms,and has good safety.

  • Clinical Observation on Huoxue Huayu Ointment(活血化瘀膏) in Treatment of Autologous Arteriovenous Fistula Stenosis

    To explore the effects of Huoxue Huayu Ointment(活血化瘀膏) on the intimal hyperplasia injury and hemodynamics,serum lipoprotein-associated phospholipase A2(Lp-PLA2),interleukin-10(IL-10) and other indicators in patients with autologous arteriovenous fistula(AVF) stenosis. Methods:A total of 90 patients with AVF stenosis were selected and divided into a control group and an observation group according to the random number table method,with 45 cases in each group. The control group was given external application of Mucopolysaccharide Polysulfate Cream,while the observation group was given external application of Huoxue Huayu Ointment. One course of treatment was 4 weeks,and a total of 3 courses of treatment were given. The patients were followed up for 6 months. The AVF patency rate of the two groups was compared. The hemodynamic indicators(blood flow,inner diameter of blood vessels) before and after treatment in the two groups were detected,and the endometrial thickness before and after treatment in the two groups was measured. And various laboratory indicators [Lp-PLA2,IL-10,high-density lipoprotein(HDL),neutrophil-to-lymphocyte ratio(NLR),platelet-tolymphocyte ratio(PLR),triglycerides-glucose index(TyG)] before and after treatment were compared. Results:Three months after the treatment,the patency rate of AVF in the observation group was 66.67%,which was higher than 40.00% in the control group(P<0.05). Six months after treatment,the patency rate of AVF in the observation group was 75.56%,which was higher than 51.11% in the control group(P<0.05). Three months after treatment,the blood flow and inner diameter of blood vessels in both groups increased compared with those before treatment,and the observation group was higher than the control group(P<0.05). Three months after treatment,the endometrial thickness in both groups was lower than that before treatment,and the thickness in the observation group was lower than that in the control group(P<0.05). Three months after treatment,the levels of serum Lp-PLA2,NLR,PLR and TyG in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05). Both IL-10 and HDL were increased compared with those before treatment,and the observation group was higher than the control group(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). The total effective rate of the observation group was 68.89%,which was higher than that of the control group at 46.67%(P<0.05). Conclusions:Huoxue Huayu Ointment can significantly increase the patency rate of AVF by inhibiting inflammation through multiple targets,improving hemodynamics,and reducing intimal hyperplasia injury.

  • Analysis of Application of “Paired Needle-Knife Method” in Treatment of Chronic Non-specific Low Back Pain

    Based on the theory of meridians in traditional Chinese medicine and the clinical practice of needle-knife,this article systematically expounds the theoretical system and diagnosis and treatment plan of “paired needle-knife method” in the treatment of chronic non-specific low back pain(CNLBP). It is proposed that the core pathogenesis of CNLBP is deficiency of the Conception and Governor Meridians and an imbalance between yin and yang,the key lies in the deficiency of Yangming Meridian and the weakness of tendons and muscles. Under the theoretical guidance of the paired acupuncture theory,the therapeutic principles of regulating the Governor and Conception Meridians,strengthening the Yangming Meridian,and unblocking meridians are established to achieve the coordinated regulation of the abdomen and back and restoring the yin-yang balance. Integrated with modern anatomy and biomechanics,the study further elucidates the mechanism by which this therapy restores spinal stability through modulating the functional balance of core muscle groups and fascial tension. A comprehensive diagnosis-treatment system from theory to practice is constructed,offering a new perspective for the needle-knife treatment of CNLBP. One test case is attached.

  • PENG Peichu's Experience in Treatment of Advanced Prostate Cancer by Integrating Cold and Heat Therapies

    This article summarizes Professor PENG Peichu's experience in treating advanced prostate cancer by integrating cold and heat therapies. Professor PENG believes that the pathogenesis of advanced prostate cancer is characterized by asthenia in origin and asthenia in superficiality,especially with deficiency of both yin and yang and insufficiency of the spleen and kidneys as the root causes,and cold and dampness stagnation and accumulation of cancer toxins as the symptoms. The mixed pathogenesis of deficiency and excess results in the syndrome characteristics of cold and heat. In treatment,the combination of cold and heat therapies should be emphasized.     This article summarizes Professor LIU Likun's experience in treating nasopharyngeal carcinoma with Mahuang Shengma Decoction(麻 黄 升 麻 汤). It is believed that this disease is characterized by deficiency of both qi and yin as the root cause,with phlegm,blood stasis,heat and toxins as the manifestations. The key to treatment lies in tonifying qi,nourishing yin and strengthening the body 's resistance,while addressing the pathological factors of phlegm,blood stasis,heat,and toxin. Based on clinical experience,it is believed that the Mahuang Shengma Decoction,which combines cold and heat and has the functions of tonifying deficiency and clearing excess,precisely aligns with the pathogenesis of nasopharyngeal carcinoma. In clinical practice,Mahuang Shengma Decoction is used as the basis for treating nasopharyngeal carcinoma. According to the patient's condition on deficiency and excess,the proportion of heat-clearing and warming medicinals in the formula is adjusted. Based on the degree of the patient's pathogenic qi stagnation,the dosage of Mahuang(Ephedrae Herba) is skillfully used. Based on clinical symptoms,channels-based differentiation and treatment is given,and the dosage of each medicinal is adjusted according to the syndrome to achieve the effects of promoting the circulation,relieving obstruction and depression,and regulating cold and heat simultaneously,tonifying deficiency and clearing excess,and treating both symptoms and root causes.     Professor QI Yuanfu believes that the pathogenesis of triple-negative breast cancer(TNBC) is closely related to “liver Xuanfu-qi and fluid-nodules”. The fundamental cause of TNBC is liver deficiency and stagnation. The disharmony of qi and fluid is the central link in the formation of TNBC,and the endogenous growth of nodules is the pathological outcome of the occurrence and development of TNBC. By analyzing the key pathogenesis of liver deficiency and stagnation,disharmony of qi and fluid,and internal generation of nodules in TNBC,as well as the evolution process of its pathogenesis from “ deficiency and stagnation → qi depression → phlegm and blood stasis”,it is proposed to treat TNBC with the general principle of “opening the liver Xuanfu and eliminating the mass”.   • Analysis of Characteristics of SONG Ruohui in Diagnosing and Treating Chronic Rhinosinusitis

    Chronic rhinosinusitis is a common disease in otorhinolaryngology. Western medicine mainly treats it with symptomatic drug therapy and surgical intervention. Professor SONG Ruohui believes that this disease is caused by insufficiency of the body 's healthy qi,invasion by external pathogens,resulting in internal heat in the viscera,which steams body fluids into phlegm and nasal discharge,blocking the meridians and impeding the flow of qi and blood. If it is not cured for a long time,the pathogenic factors will linger and gradually affect the spleen and kidneys,causing their deficiency and leading to the disease. It is pointed out that the disease is located in the lungs,but is closely related to the liver,gallbladder,spleen and kidneys. Therefore,in terms of etiology,Professor SONG emphasizes that the lungs and liver-gallbladder should be held primarily responsible to clarify the source of pathogenic factors,followed by examination of the deficiency or excess of the spleen and kidneys to judge the changes in the condition,and then exploration of the nature of the pathogenic factors,whether they are cold or hot,and their severity. In treatment,Professor SONG advocates treating the gallbladder and lungs simultaneously,using both clearing and promoting methods;cooling the blood and promoting blood circulation,eliminating dampness and reducing swelling;tonifying qi and strengthening the body's foundation,while also taking into account yin and blood. In clinical practice,Professor SONG applies flexible and adaptable methods,treating each patient individually.   • Test Cases of Modified Yunü Decoction(玉女煎) in Treatment of Head and Facial Dermatosis of Yangming Heat Exuberance Type

    There are numerous types of skin diseases on the head and face. From the perspective of traditional Chinese medicine's physiological characteristics,the head and face are the convergence points of all yang qi,and the Yangming meridian is widely distributed. The vitality of the face depends on the prosperity and decline of the qi and blood in the yang meridians,especially the Yangming meridian. Excessive heat in the Yangming meridian is a common traditional Chinese medicine(TCM) syndrome type of head and facial skin diseases,and it is essentially consistent with excessive heat in the lung and stomach. Yunü Decoction(玉女煎) can clear excessive heat in the Yangming meridian and nourish the deficiency of water in the Shaoyin meridian. After clinical modification,it can be applied to the treatment of head and facial skin diseases. This article presents three test cases of Modified Yunü Decoction in the treatment of head and facial skin diseases,including acne,facial contact dermatitis,and rosacea. During the clinical diagnosis and treatment process,the main approach is to clear excessive heat in the Yangming meridian,while also taking into account nourishing yin,promoting blood circulation,eliminating dampness,and clearing the liver,which achieves remarkable therapeutic effects.
