芪归药对治疗气虚血瘀型糖尿病肾病临床研究
Clinical Study on Qigui Medicinal Pair in Treatment of Diabetic Nephropathy of Qi Deficiency and Blood Stasis Type

摘要(Abstract):

目的:观察芪归药对治疗气虚血瘀型糖尿病肾病的疗效及对患者超氧化物歧化酶(SOD)、过氧化氢酶(CAT)的影响。方法:将符合标准的60例患者以随机数字表法分为观察组和对照组各30例。两组均采用基础治疗,观察组在此基础上加服芪归药对颗粒剂(黄芪30 g,当归20 g),疗程12周。比较两组治疗前后的中医证候积分,以及空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbAlc)、肾小球滤过率(eGFR)、血肌酐(Scr)、SOD、CAT的水平,并统计不良反应。结果:与治疗前比较,治疗后两组患者中医证候积分、FPG、2 h PG、HbAlc、eGFR、Scr、SOD、CAT均较前改善(P<0.05),且观察组上述指标改善效果优于对照组(P<0.05)。两组均未见明显不良反应。结论:芪归药对治疗气虚血瘀型糖尿病肾病安全有效,其机制可能为通过上调SOD、CAT活性进而调控氧化应激,保护肾脏功能。
Objective:To observe the curative effect of Qigui medicinal pair on diabetic nephropathy of qi deficiency and blood stasis type and its influence on superoxide dismutase( SOD) and catalase( CAT).Methods:Sixty patients meeting the criteria were divided into observation group and control group according to the random number table method,30 patients in each group. Both groups were given conventional treatment, and the observation group was further treated with Qigui medicinal pair granules [ Huangqi( Astragali Radix) 30 g,Danggui(Angelicae Sinensis Radix) 30 g].One treatment course lasted 12 weeks. Traditional Chinese medicine(TCM) symptom score,fasting blood glucose( FPG), 2-hour postprandial blood glucose(2 h-PG),glycosylated hemoglobin(Hb Alc),glomerular filtration rate(e GFR), serum creatinine(Scr), SOD and CAT levels were compared between before and after treatment,and adverse reactions were also recorded. Results:Compared with before treatment,in both groups after treatment TCM symptom score,FPG,2 h-PG,Hb Alc,e GFR,Scr,SOD,and CAT were all improved(P<0.05),especially in observation group(P<0.05). No significant adverse reactions occurred in either group.Conclusion:Qigui medicinal pair is effective and safe in treating diabetic nephropathy of qi deficiency and blood stasis type. The possible mechanism might works through regulating oxidative stress by up-regulating SOD and CAT activity to protect kidney function.
